To submit a form, please print the form, complete and sign it, then fax it to us at 614-781-7200.  Or, you can email a scanned version of the completed form to Christie Davis at cdavis@sumtotalsystems.com.  An Account Manager will begin processing your request promptly.

Expiration Code Request
This form allows you to give us the information we need to issue you the appropriate Expiration Code (EXPID). For your convenience, the expiration date for the EXPID is aligned to the end of your maintenance contract.

Important: Before you complete the request form, please take a minute to review the different types of Expiration Code Requests. Doing so will help avoid delays in receiving your Expiration Code.
